FALL PREVENTION
Our medical professionals will perform a Home Safety Evaluation to identify risk factors in the home. Changes in health conditions, advancing age and certain medications can increase the risk of falls. Safety measures will be implemented to achieve a safe living environment. Our therapist will also develop a home exercise program to increase balance and overall strength to prevent falls or injury for those in need.
MEDICATION MANAGEMENT
Our nurses work closely with patients and physicians to achieve desired medication effects and to prevent medication interactions. Education will be provided on therapeutic effects, adverse effects and common side effects of all medications. Testing for therapeutic levels can also be performed as requested by physicians. Our nurses will develop medication schedules or weekly planners to increase adherence to medication regimens.
WOUND CARE/POST-OPERATIVE CARE
Our nurses provide quality wound care with the most innovative supplies in the industry. Our nurses received extensive training on Negative-Pressure Wound Therapy and wound care techniques throughout the year. Our excellent Wound Care Program has been successful in healing of chronic wounds.
PREVENTING HOSPITALIZATION
Our clinicians will make every effort possible to prevent re-hospitalization. Education, medication reconciliation and certain procedures provided in the home have proven to be effective in preventing re-hospitalizations. We have maintained lower rehospitalization rates among our patients compared to other home health’s in the nation as supported by Kinnser RiskPoint.
CHRONIC CARE
Our dedicated clinicians specialize in individualized care plans to manage new or chronic illnesses (Alzheimer's, arthritis, congestive heart failure, COPD, diabetes, dementia). We provide thorough and innovative information on illness management. Optimal independence will be achieved with the participation of patients and caregivers. Our desire is for patients to achieve the highest quality of life possible using a multidisciplinary and holistic approach.
